Accessibility & Technical Assistance Templates

Self-advocacy templates in 3 big clicks — faster than a generic chatbot: answer a short needs questionnaire once, then reuse your light profile on HUD/FW/SSA/CRD plus DOR intake, DOR appeal, ADA accommodation, business-start, and mediation templates.

These are free self-advocacy templates from Amplify Accessibility, a 501(c)(3) nonprofit. They include a housing reasonable accommodation letter (HUD has no official form for this), court fee waiver FW-001 help for California, DOR intake and appeal letters, and ADA accommodation request letters that you can preview, personalize, and download, with non-attorney help rather than legal advice.

Amplify Accessibility Green Tech Coalition is an IRS-recognized 501(c)(3) public charity (EIN 99-3298727).

Questions about free accommodation templates

How do I get help filling out a disability accommodation form?

Open Ask EMILY (the button at the bottom-right of every page on amplifyaccessibility.org) and type or speak your need. EMILY helps you fill template paths such as a housing reasonable accommodation letter, FW-001 court fee waiver, and SSA-561, or drafts a custom accommodation request letter that you can sign on screen (drawn or typed signature) and download or print. It is free, non-attorney administrative help, not legal advice.

Where can I find a housing reasonable accommodation letter?

In step 1 on this page, choose “Housing reasonable accommodation letter (HUD has no official form)” to preview and download our free Housing Reasonable Accommodation Request Letter outline (there is no official HUD form for this). Your saved profile fills in your name and goal, and EMILY can help you finish and sign it. It is a plain-language self-advocacy letter, not a government form or legal advice.

How do I request a Section 504 reasonable accommodation?

Write to the school, program, housing provider, or employer that receives federal funding, and ask for its Section 504 coordinator. Say that you have a disability, describe the barrier, and name the specific accommodation you are requesting. Date the letter, keep a copy, and ask for a written response. Our free self-advocacy templates and EMILY can help you draft it.

Where can I get court fee waiver FW-001 help in California?

Form FW-001 (Request to Waive Court Fees) asks a California court to waive filing fees if you receive certain public benefits, have a low income, or cannot pay for basic needs and court fees. Choose FW-001 in Start Assistance and EMILY helps you organize your information; you file the official Judicial Council form with your court, and the court’s self-help center can review it.

Can I get help with a California Department of Rehabilitation (DOR) appeal?

Yes. If you disagree with a DOR decision, you can ask for an informal administrative review, mediation, or a fair hearing, and the Client Assistance Program (CAP) can advise you at no cost. Our DOR-APPEAL template helps you organize the facts and write a clear request. It is self-help, not an official DOR form or legal advice.

Is Miracle Spritz safe for assistive technology and wheelchairs?

The Miracle Spritz multi-surface cleaner MSDS (dated 08/16/2022) lists an HMIS rating of 0-0-0 for health, fire, and reactivity, 0% VOCs, and no flash point. It is a ready-to-use spray for hard surfaces, including screens, electronics, and mobility hardware such as wheelchairs. Follow your device or wheelchair maker’s cleaning guidance. See our Safety Data Sheets.

Is this legal advice?

No. Amplify is not a law firm. These templates are an accessibility & communication accommodation tool under Section 504 / Section 508 / ADA Title II/III — not legal counsel. For representation, contact Central California Legal Services or the Fresno County Bar Association.

Are downloads free?

Yes. Free informational resources from our 501(c)(3). Donations welcome but not required.

Where does my profile go?

Your light advocacy profile is stored in this browser under amplify_advocacy_profile so EMILY’s Start Assistance tab and these templates can reuse name, email, need type, and goal. We do not open private EMILY source systems from this page.
